Look for specially marked products with a hang tag attached (You will need this in order to submit a specific code for your rebate). When you mail in your receipt with this form you can choose to get a magazine subscription or check "No Thanks" and receive a refund of $12.00 (value of the subscription)! Read the fine print on the form for further details and to make sure you include everything when mailing in your submission.
